XCaliber Health is an agentic operating system for healthcare, led by co founder and CEO Prakash Khot, previously a co founder of the AI data control company Skyflow, and backed by a six and a half million dollar seed round announced in May 2026 led by ManchesterStory with Benhamou Global Ventures and Arka Venture Labs. Headquarters is reported as Andover, Massachusetts in the announcements and as Plano, Texas by Crunchbase. The premise: for two decades health systems accumulated electronic health records, billing platforms, and scheduling tools without ever acquiring a layer to coordinate across them, leaving humans to stitch systems together and the average physician losing more than fifteen hours a week to administrative work.

XCaliber positions itself as that coordination layer, a single intelligence layer where data, agents, and knowledge converge on top of the fragmented stack a provider already depends on rather than replacing it. Merlin is the role based AI agent line, and Patient Navigator, the first shipped functioning agent, spans scheduling, prescription refills, outreach, intake, referrals, prior authorization, and billing resolution. The architecture is deliberately semi autonomous: agents run routine administrative and operational workflows end to end and take action across silos in real time, while a human in the loop structure keeps clinical and operational teams in control of every critical decision, with the degree of autonomy varying by whether a task is operational or clinical. Agents are grounded in the individual healthcare system's own data, and the platform blends generative AI with traditional machine learning and microservices so that deterministic tasks stay deterministic. The company reports the platform already processes more than eight million chart updates and generates more than one hundred sixty thousand EHR updates daily across more than seven hundred thousand unique patients, with the patient navigator agent saving providers an average of six hours of manual work per day on prescription refills alone.

XCaliber fits ambulatory practices, health systems, provider groups, and digital health partners that want to coordinate an existing EHR and operations stack with agents rather than add another point solution. It competes with Notable Health and Ambience Healthcare, and is a weaker fit for buyers needing published pricing or self hosted deployment.
